SCHOOL HEALTH SERVICES and INFORMATION
In a partnership with the Cumberland Family Medical Center, we have established satellite Child Health clinics in each of our five school buildings.  Parents may make appointments for school physical exams and immunizations.  The nurses also provide case-management of chronic health conditions.  If your child has been diagnosed with an illness that requires a health service during school hours, please contact the nurse at your child's school or the school health coordinator.

 

HEALTH RECORDS REQUIRED FOR ENROLLMENT

To enroll a child in the McCreary County School System, you must provide the following health documents to the school:

• Valid Kentucky Immunization Certificate 1         New shot requirements effective July 1, 2011
• Preventative Health Care Examination Form (KDE form)2
• Kentucky Eye Exam Form for School Entry  (for 3, 4, 5, and 6 year olds)3
• Kentucky Dental Screening/Examination Form for School Entry  (for 5 and 6 year olds)4

 

If your child needs to receive an immunization, or "booster shot," check with the nurse at the school that your child attends.  The nurse can tell you what is available and make an appointment for you at the school clinic.

1Kentucky immunization requirements were recently revised for children entering day care, Head Start programs, public and private preschool programs and public and private schools.  The effective date for the new changes is July 1, 2011.

2A Preventative Health Care Examination is required for each child within one (1) year prior to the child's initial admission to school.  A second examination is required within one (1) year prior to entry into the sixth grade.  These exams must be reported on KDE forms.

3Kentucky law, KRS 156.160 (1) (g), requires proof of a vision examination by an optometrist or ophthalmologist. This evidence shall be submitted to the school no later than January 1 of the first year that a three (3), four (4), five (5) or six (6) year old child is enrolled in public school, public preschool, or Head Start program.

4Kentucky law, KRS 156.160(i), requires proof of a dental screening or examination by a dentist, dental hygienist, physician, advanced registered nurse practitioner, physician assistant, or Kentucky health department KIDS Smiles nurse.  This completed exam form (OH-12,rev 1/25/11) shall be presented to the school no later than January 1 of the first year that a five (5) or six (6) year old is enrolled in public school.
